Ride Forms

The Chapter's goal is to ride and have fun.  So that guests and minors can join in the fun and enjoy the ride.  We do ask the following forms be completed and presented to the lead Road Captain.

Some suggestions as to how we like to ride as a chapter.

Chapter Event Release Form for Minors

Legal Waiver form to be completed by a non-adult rider's parent or legal guardian.

Chapter Event Release Form for Adults

Legal waiver form to be completed by an adult guest

Minor's Assumption of Risk Acknowledgement

If the minor is older than 12 years old, they need to read and sign this form.
